Male Breast Reduction: Plastic Surgery for Gynecomastia

What is Gynecomastia?

Gynecomastia is an extremely common occurrence in men.  Many may notice localized breast fullness or swelling around puberty, while the majority will develop excess breast tissue and fat associated with weight gain or aging.  When gynecomastia is present in youth or develops suddenly, a thorough medical evaluation for the causes of the gynecomastia is warranted.  Abnormalities of the pituitary gland or testes may cause gynecomastia. Marijuana and other recreational drugs may cause gynecomastia.  Many other drugs can cause gynecomastia.  In the vast majority of cases, there is no identifiable cause for male breast enlargement.  When the “gynecomastia” is unilateral (on one side), this demands a more urgent and thorough evaluation to rule out male breast cancer.

Gynecomastia can be the source of considerable self consciousness for men.  Many men with gynecomastia have tried weight loss, pectoral training, and other exercises to minimize the male breast enlargement. Men with gynecomastia are reluctant to take their shirts off in public, and often choose shirts that conceal the prominence of their breasts.

Plastic Surgery for Gynecomastia

The plastic surgery techniques for treating gynecomastia are different, and are selected based on the individual’s needs.  Gynecomastia may involve excess breast gland, excess breast fat, excess breast skin, or any combination of the three.

When breast fat alone predominates, and when skin elasticity is good, liposuction is often the treatment of choice for this type of gynecomastia.  When there is a combination of excess fat and breast gland, the combination of liposuction and direct gland removal through a small incision in the areola is appropriate.  Gynecomastia that is associated with excess skin may require additional incisions.  Whereas a certain amount of skin contraction or shrinkage is anticipated after gland and fat removal, when there is excessive inelastic skin in gynecomastia, skin reduction with incisions around the areola, or potentially in the infra-mammary crease in more extreme cases, is required.

What to anticipate after Gynecomastia correction

After the gynecomastia surgery, small drain tubes are often placed that are usually removed three or four days after the procedure. A compressive vest is worn for anywhere from 2 to 6 weeks post-op. Return to work is often three to five days after gynecomastia surgery, and exercise may resume at one week.

Which gynecomastia surgery technique is best for me?

After a comprehensive evaluation by a board certified plastic surgeon, looking into any potential correctable causes of gynecomastia, and assessment of the balance of gland, fat, and / or skin excess, a treatment plan for male breast reduction that suits you best can be formulated.
